Contract dentist - apply for authorisation
As a dentist, you can treat people with statutory health insurance and bill your treatment to the health insurance fund. To do this, you must have a licence to provide contract dental care.
This authorisation makes you a member of the Association of Statutory Health Insurance Dentists (KZV). This means that you are authorised and obliged to participate in the provision of care in accordance with your full or half of your care mandate.
Please note: The admission restrictions for contract dental care were lifted on 1 April 2007.

Prerequisite
You can only be authorised to practise as a contract dentist if you
- are entered in the dental register,
- are not prevented from providing the necessary care to insured persons by employment or other non-voluntary activities (maximum 13 hours per week for full authorisation),
- only carry out dental activities that are compatible with the activities of contract dentists and
- have no health or other serious personal deficiencies (in particular, you must not have been addicted to drugs or alcohol in the five years prior to submitting your application)
Note: You can be authorised as a contract dentist even if there are reasons for obstruction as defined in the second or third bullet point. However, the relevant impediment must be removed no later than three months after the licence has become unappealable. Activities in authorised hospitals, preventive care or rehabilitation facilities are compatible with contract dentistry.
Procedure
You must apply for authorisation in writing to the competent authority and pay the fees due.
The application must include
- the location and
- if applicable, for which speciality you are applying for admission
The authorisation committee decides on the application after an oral hearing. The decision will specify the date by which you must commence practising as a contract dentist at the contract dentist's practice.
Note: If there are important reasons, this date can be postponed on request.
The decision will be sent to you together with a description of your rights. The Association of Statutory Health Insurance Dentists and other parties involved will also receive a copy of the decision.

Required documents
- Extract from the register of dentists, which shows:
- Date of licence to practise
- Date of entry in the dental register
- the date of recognition of the right to use a specific speciality title, if this has been granted
- Certificates of the dental activities carried out since the licence to practise dentistry
- a declaration that you intend to reduce the scope of care to half, if you intend to do so
- Curriculum vitae
- Certificate of good conduct
- if applicable, certificates from the Association of Statutory Health Insurance Physicians (KZV) in whose area you were previously established or authorised to practise (stating the place and duration of the establishment or authorisation and the reason for any termination)
- Declaration of service or employment relationships existing at the time of application (stating the earliest possible end of the employment relationship)
- Declaration as to whether you
- are addicted to drugs or
- have been addicted to narcotics within the last five years or
- have undergone rehab treatment for drug or alcohol addiction within the last five years and
- that there are no legal impediments to practising the dental profession
You must submit the documents in the original or as a notarised copy.
Please note: If you are unable to present the above-mentioned certificates of your dental activities to the authorisation committee, you must provide other evidence of these circumstances. Please enquire about this at the relevant office of the authorisation committee.
Costs
- for the application for authorisation (due with the application): EUR 100.00
- Additional administrative fee after authorisation has become unappealable: EUR 400.00
Processing time
- Submission of the complete application documents to the office: approximately six weeks before a meeting of the admissions committee
- Meeting dates: Mid-month of the last month of a quarter
Miscellaneous
As a contract dentist, you must hold your consultation hours at the contract dentist's practice.
Contract dentist activities outside the contract dentist's practice are permitted if and insofar as this improves the care of insured persons at other locations and does not impair the proper care of insured persons at the location of the contract dentist's practice. Minor impairments to care at the location of the contract dentist's practice are irrelevant if they are outweighed by the improvement in care at the other location.
If the branch practice is located in the area of your member KZV, it is responsible for authorising a corresponding application. Otherwise, authorisation is granted by the licensing committee in whose district the branch practice is located.
If you wish to relocate your contract dental practice, you must submit an application to the authorisation committee.
